We replaced a heavily customized legacy EHR with a unified platform to enhance clinical operations, patient digital services, and centralized management reporting across a multi-site clinic network.
A privately held clinic network operates multiple locations across a European country. Most clinics are concentrated in one major city, with additional sites in other urban centers. The organization provides outpatient services and operates under national regulations governing patient data, medical records, and information security.
Previously, limited investment in IT modernization had left the network dependent on a highly customized legacy platform built around existing processes. After a change in ownership, the new management launched a broader modernization program aimed at improving business operations, expanding digital services for patients, and replacing the legacy technology stack.

The clinic network relied on a legacy EHR that had been extensively customized over many years. Over time, maintaining and extending the system became increasingly expensive. Moreover, the platform lacked capabilities expected from a modern healthcare organization, such as online appointment booking, patient self-service, digital payments, and centralized reporting.
The legacy environment also limited the network’s ability to grow, due to constraints around third-party integrations, manual processes, and inconsistent capacity planning.
Following the change of ownership, the organization needed to modernize its operations and technology foundation. Unlike implementing a new system in a greenfield environment, the project required replacing a platform deeply embedded in daily clinical workflows while maintaining uninterrupted patient care across a multi-site network.
Before implementation, the organization needed to determine whether to continue with a custom-built solution or adopt a commercial platform. Budget constraints, data migration from the legacy system, integration with existing third-party applications, compliance with national health data regulations, and the requirement for uninterrupted clinical operations all had to be considered before a technology decision could be made.
EffectiveSoft supported the initiative from platform evaluation through implementation planning, configuration, integration, migration, and rollout.
The initiative combined platform selection with a phased implementation program designed to replace the existing EHR while keeping clinical operations running throughout the rollout.
We began by defining business and technical requirements, evaluation criteria, budget constraints, and implementation priorities. EffectiveSoft supported vendor scouting, prepared the RFP documentation, coordinated demonstrations, and evaluated commercial platforms alongside the option of building a custom EHR solution.
Following the evaluation, the team recommended a commercial platform. The decision was driven by the following factors: the available budget, required delivery timeline, and the level of functionality already available in commercial platforms.
EffectiveSoft also prepared the implementation roadmap, budget estimates, and supporting materials for management review before the project moved into delivery.
The selected commercial platform was configured for the clinical workflows, customized where standard functionality did not cover specific requirements, and integrated with existing third-party systems.
The new platform introduced patient-facing capabilities that had not previously existed across the network. Patients gained access to digital services such as online appointment scheduling, secure messaging, test result access, online payments, and family charts. At the same time, the organization gained a more consistent way to manage interactions across its locations.
The implementation also established a centralized reporting environment. Data from the EHR, scheduling systems, and commercial operations was consolidated into BI dashboards, replacing manually prepared operational and financial reports with centralized reporting.
Data was migrated from the legacy EHR despite years of customization. To reduce operational risk, the rollout followed a pilot-first approach. A smaller clinic was selected for the initial deployment, allowing the project team to validate the migration process, configuration, integrations, and operational workflows before proceeding with the remaining locations.
Following the pilot and go/no-go decision, the rollout expanded across the network using a phased deployment approach tailored to the client’s operating model.
User training accompanied each deployment phase, while hypercare support helped clinical and administrative teams transition to the new system.
Because uninterrupted patient care was a mandatory requirement, legal and medical leaders participated in risk management throughout the rollout. The delivery team prepared rollback procedures, monitored system availability during cutover activities, and addressed issues as they arose.
The clinic network moved from a heavily customized legacy EHR and manual reporting processes to a unified platform supporting clinical operations, patient services, and management reporting across all locations, with no significant interruption to patient care during rollout.
